All medications must be picked up from the school clinic by 12:00pm on June 12. Medications not picked up will be discarded. No medications can be kept in the school clinics over the summer.

 New Medication Administration Forms and/or Health Treatment Plans are required each school year. Forms may be printed from your  SCHOOL NURSE'S WEBPAGE .

 

IMPORTANT IMMUNIZATION REQUIREMENTS FOR SY 26-27

Students who do not meet the immunization and /or physical examination requirements of the current school year will be excluded from school until the requirements are met.
